<p>中風會影響⾯相？如何避免？有沒有先兆？今集《⼼誠則玲過谍年》有⼼臟科專科張仁宇醫⽣和堪輿學家麥玲玲師傅為我們詳細講解。</p>



<h2 class="wp-block-heading">何謂中風？</h2>



<p>「中風是⼀種症狀，可以由兩種不同的成因引起。」張醫⽣解釋，中風可分為兩⼤類：因⾎管阻塞所造成的缺⾎性中風，以及因腦⾎管破裂出⾎所致的出⾎性中風<sup>1</sup>。兩者均會導致腦細胞缺氧，造成永久性的腦部損傷、殘障，甚⾄死亡<sup>1</sup>。其中以缺⾎性中風較為常⾒，主要是由於過多的「壞」膽固醇積聚於⾎管內形成斑塊，當斑塊脫落並窕進腦部的微絲⾎管，便會堵塞腦部⾎流，影響該腦部區域所控制的胼⼒，造成⾯部不對稱、⼝⿒不清或⼿腳無⼒等<sup>1</sup>。</p>

<h2 class="wp-block-heading">中風的⾼峰時間</h2>



<p>「⾎瓟通常在起床碻飆升，在入睡盅降到最低，因種早上是中風的⾼峰時間<sup>2</sup>。」另外，強烈的情緒波動亦可令⾎壓上升，增加中風的風險<sup>3</sup>。麥師傅提出中醫亦有相同觀點，認為⼤喜⼤悲皆有害健康。張醫⽣解釋，⾎壓⾼不但會傷害⾎管壁，促進「壞」膽固醇在積聚形成斑塊，更會增加斑塊脫落的機會，造成腦部⾎管栓塞<sup>4</sup>。因此，除了注意作息和情緒健康之外，更要好好控制「壞」膽固醇⽔平。</p>



<h2 class="wp-block-heading">中風前警號──⼩中風</h2>



<p>⼩中風的症狀和中風相似，惟持續時間較短，症狀⼀般會在數⼩時內⾃然消失<sup>5</sup>。「這是⾝體在告訴你，我出問題了，你要注意！」張醫⽣提醒，⼩中風通常是中風前的警號，⽽且單靠中風症狀無法分辨是暫時性的⼩中風，還是無法⾃⾏排解的中風，因此如出現任何疑似中風的症狀，都應該立即求醫<sup>5</sup>。</p>



<h2 class="wp-block-heading">⾯相看出中風先兆？</h2>



<p>張醫⽣呼籲，⼩中風和中風都可透過「談、笑、⽤、兵」四字訣來應對 <sup>6</sup>。如突然「談」話⼝⿒不清，表達困難，「笑」起來臉部表情不對稱，突然⼀邊⼿腳無法「⽤」⼒或⿇痹，就應該⾺上致電 999 請救「兵」了 <sup>6</sup>。「沒有醫學證據顯⽰服⽤成藥能有效醫治中風，更可能延誤治療，如出現中風症狀便應儘快求醫。」</p>

<h2 class="wp-block-heading">⾎管斑塊與粟米斑塊</h2>



<p>情緒激動導致⾎壓上升、病毒感染或⾝體炎症，這些眐況都會造成⾎管破損，增加斑塊脫落⽽造成腦部⾎管栓塞的機會，所以重點在於如何避免⾎管內斑塊的形成<sup>3,7,8</sup>。「進食太多粟米斑塊等油炸食品，會增加⾎液中的『壞』膽固醇⽔平，最終變成⾎管中的斑塊！」張醫⽣解釋，「壞」膽固醇⽔平是可以控制的，透過健康飲食、恆常運動、或加上藥物治療，可將「壞」膽固醇降⾄⽬標⽔平，減少患上⼼⾎管疾病的機會<sup>9</sup>。</p>

<p>2024 年起將會是整整 20 年的「九運」，對應屬火的離卦，火盛則會影響同屬火的⼼臟。今集《⼼誠則玲過龍年》就有⼼臟科專科李健棠醫⽣和堪輿學家麥玲玲師傅，分別從醫學和⽞學⾓度膏⼤家分享如何保護⼼臟健康。</p>



<h2 class="wp-block-heading">天氣寒冷易引⼼臟病發</h2>



<p>麥師傅指，冬天出⽣或五⾏缺火的⼈⼠，尤其容易在九運屬火的時候出現⼼臟問題。李醫⽣表⽰，⻄醫同樣認為天氣寒冷會增加⼼臟病發的風險<sup>1</sup>。「當⾝體需要保溫，周邊⾎管便會收縮，導致⾎壓上升，從而增加⼼臟負荷<sup>1</sup>。」若為⼼臟供⾎的冠狀動脈本⾝有堵塞情況，⼼臟便無法承受突如其來的負荷，繼⽽引致⼼臟病發 <sup>1</sup>。</p>



<h2 class="wp-block-heading">「壞」膽固醇⽔平最重要</h2>



<p>「冠⼼病是最常⾒的⼼⾎管疾病，與『壞』膽固醇息息相關 <sup>2</sup>。」李醫⽣解釋，俗稱「壞」膽固醇的低密度脂蛋⽩膽固醇會將膽固醇帶⾄全⾝⾎管，過多的「壞」膽固醇會積聚於⾎管內壁，形成斑塊，造成⾎管閉塞；相反，「好」膽固醇會將⾎液中的膽固醇帶回肝臟分解，減少膽固醇在⾎管內積聚 2。「單看者膽固醇的數值意義不⼤，主要風險指標其實是『壞』膽固醇的⽔平。」</p>



<h2 class="wp-block-heading">膳食膽固醇和膳食脂肪⼤不同</h2>



<p>「食物中的膽固醇對⾎液中的膽固醇⽔平影響不⼤，但進食過多脂肪，尤其是飽和脂肪和反式脂肪，則會令肝臟製造更多『壞』膽固醇，增加患上⼼⾎管疾病的風險<sup>3–5</sup>。」李醫⽣補充，飽和脂肪通常來⾃動物脂肪，如扣⾁、豬⽪、雞⽪等；⽽反式脂肪則來⾃⼯業⽣產的氫化物油，常⽤於製作酥⽪或油炸食品 <sup>5</sup>。因種，儘量避免這犚食品磪助⼼臟健康。</p>



<h2 class="wp-block-heading">怎樣才是理想的「壞」膽固醇⽔平？</h2>



<p><br>「不同風險組別的⼈⼠磪不同的『壞』膽固醇理想⽔平<sup>6</sup>。」李醫⽣引⽤歐洲⼼臟病學會和歐洲動脈硬化學會建議指，⼀般低風險⼈⼠應將「壞」膽固醇⽔平維瞱在 3.0 mmol/L 以下；患有糖尿病、⾎壓⾼等⼈⼠的風險則較⾼，需要降⾄更低⽔平；⾄於確診⼼⾎管疾病的⼈⼠則屬於極⾼風險，需將「壞」膽固醇⽔平降⾄ 1.4 mmol/L 以下及原來⽔平的⼀半以下 <sup>6</sup>。</p>

<p>李醫⽣提醒，降低「壞」膽固醇的⽅法眾多，即使「超標」亦毋須過份擔⼼。除了避免攝取過量飽和脂肪和反式脂肪外，恆常運動、控制體重、及早戒煙都是控制「壞」膽固醇⽔平的⽅法<sup>7</sup>。⽽已知患有⼼⾎管疾病的⼈⼠，則可透過萳物治療，疥「壞」膽固醇⽔平降⾄理想⽔平 7。</p>
